In Missouri you do not need a doctor's referral to start.

Most people out here still believe they do, and wait until the thing that would have taken six weeks takes six months. You can ring us directly. Here is exactly how far that goes before a doctor has to be involved.

A doctor is involved

At that point we consult your physician to keep going. If you are not measurably better by then we refer you on, which is the right outcome anyway.

Missouri Revised Statutes section 334.506, as amended effective 28 August 2023. A therapist may treat without a prescription if they hold a Doctor of Physical Therapy degree or have at least five years of clinical practice. One caution worth stating plainly: Medicare generally still expects a physician signed plan of care for continuing coverage, so if Medicare is paying, expect to involve your doctor sooner than the statute alone requires.

Five things you can test at your own kitchen counter

Before you ring anybody. None of these is a diagnosis. They are the same screens we would start with, and they will tell you whether what you have is settling on its own or is the kind of thing that gets worse by waiting.

Stop any of these if it produces sharp pain, dizziness, numbness or a giving way. Do them near a counter or a wall you can catch. If you have had a fall, a fracture, recent surgery, chest pain or new weakness in a limb, skip all of this and telephone a doctor today.

Sit to stand, five times

From an ordinary kitchen chair, arms folded across your chest, stand fully and sit fully five times as quickly as you safely can. Time it.

  • Under 12 seconds. Leg strength and control are holding up.
  • Over 15 seconds, or you need your arms. Worth an appointment. This is the single best predictor of a future fall we have.

Stand on one leg

Hands at your sides, eyes open, one foot off the floor, next to the counter but not touching it. Time both sides separately.

  • Ten seconds or more, both sides. Balance is doing its job.
  • Under five seconds, or one side much worse. A difference between sides matters more than the number.

Reach behind your head, then behind your back

Reach over your shoulder as if scratching between your shoulder blades, then reach up your back from below. Compare left with right.

  • Both sides roughly equal. The shoulder is moving through range.
  • One side stops well short, or the shoulder hurts at night. Night pain in a shoulder is the symptom people ignore longest.

Bend forward, then lean back

Standing, bend slowly toward your toes and come back up, then place your hands on your hips and lean gently backward. Note which direction is worse.

  • Stiff but easing after a few repeats. A mechanical back that usually responds well.
  • Pain travelling below the knee. Different problem. Bring it in rather than waiting it out.

Walk a straight line down the hall

Ten paces at your normal speed with somebody watching, then ten back. Ask them what they see rather than how it feels.

  • Even, quiet, arms swinging. Nothing to chase.
  • A limp, a dip, or one arm not swinging. A watcher spots this in seconds and you will never feel it yourself.

What actually walks through the door in Ray County

  • Shoulders and backs off the plant floorRepetitive lifting and overhead work. These arrive late, because people assume it is just the job.
  • Farm injuriesOverexertion, a fall from a step or a gate, a knee twisted on uneven ground. Rarely reported to anybody, often carried for months.
  • High school sportAnkles, knees and shoulders from football, wrestling and track across every town in the county.
  • New knees and hipsPost operative rehabilitation after joint replacement, which is a course of work with a known end point rather than an open ended arrangement.
  • Balance and fallsThe largest group and the one that benefits most from arriving before the fall rather than after it.

The first visit

How longFifty five minutes, and you are with the therapist for all of it rather than being handed to an aide after ten.
What to bringYour insurance card if you have one, a list of your medications, and the shoes you actually wear. No referral needed to start.
What to wearSomething you can move in and that lets us see the joint. For a knee, shorts.
What you leave withA written plan with the number of visits we expect, and two or three things to do at home. If we think you do not need us, we will say so at the first visit.
If you have no insurance$120 for the evaluation and $85 a visit after it, paid on the day. That is the whole price list.
Waiting timeNew patients are usually seen within three working days. Post operative referrals from the hospital are seen within one.

If one of the five tests above came out badly, that is reason enough to make the appointment. You do not need permission from anyone to start, and if it turns out you need a doctor rather than a therapist, telling you that is part of the job.
